Referral Bonus:   $100 Gift Card – For a direct introduction to a dentist

To Qualify:

• The referring team member must complete the Connect & Earn Form (Referral Bonus Form) – A link to this form is available by clicking the “Refer a Dentist” button at the top of the page in Empower.
• The referring team member must facilitate a personal introduction between the dentist and the Apex affiliations team (i.e., not a general practice number to call).
• The referral must represent the affiliation candidate’s first contact with Apex.
• The first person to refer the dentist will be the only team member eligible to receive the referral bonus.
• The referring team member must be employed by an Apex-affiliated practice at the time of the introduction to receive the referral bonus.
• All referral bonus payments will be paid within 30 days of the first call between the dentist and Apex.
